Resale/Exemption Certificate

We require a completed Resale/Exemption Certificate form from you. We are unable to accept W9 forms, Business Permits/Business Registration Certificates, or Articles of Incorporation, as proof of tax exemption.

Please e-mail a scanned copy of your supporting registration. Please ensure that it is readable - the text must be clear, the entire certificate must be captured and the signature must be included to be valid.
